Our Insurance guy will be at practice on Saturday, February 24. If you do not have insurance you must purchase it at this time. You will need to have either $16 (without 50K death benefit) or $20 (with 50K death benefit) cash or check for the first months premium due on the 24th. Also a voided check and a date to have automatic withdrawal done each month thereafter. If you do not have a checking account you will have to make arrangements with him at this time.

You may also pay all up front $104 without death benefit or $130 with death benefit for 6 months coverage. This also needs to be paid by check or cash. He will be there on the 24th at 10am.

If you currently have your own insurance, you must bring a copy for Doc Mike to keep on the 24th. You will not be issued your equipment without insurance.

Please keep in mind, this gentleman is doing us a favor and makes little to no money from these policies. Be prepared with payment in full or a checking account to debit monthly. You cannot be billed for the monthly premium as this cost him more money and these are the lowest premiums available. Let's look professional so he returns next year!
