Combined insurance will be at practice Tues March 4th at 6:30PM. You may purchase accidental insurance for $16/month, or $20/month based upon coverage. If you prefer to pay all up front be prepred to pay 6 months premium. You may have this charge debited from an account monthly if you bring a check for processing. Should you pay monthly the first payment is due at the time of signing. Policies become effective immediately. Please have available work address in addition to home and birthdate. This is great coverage for a minimal fee and coverage is required by the Phoenix Managment prior to playing.
